7/12/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning observations, the client exhibited difficulty remaining focused and following directions while eating breakfast with her peers. During midday observations, the client exhibited difficulty communicating/responding to authority in an appropriate manner and remaining focused while watching a movie. During afternoon observations, the client kept her hands and feet to herself when interacting with her peers while cleaning up the room. QMHP provided Therapeutic Day Treatment services for the client throughout the day. During morning observations, QMHP redirected the client and reiterated the importance of remaining focused and following directions. During midday observations, QMHP redirected the client and reiterated the importance of communicating/responding to authority in an appropriate manner and remaining focused. During afternoon observations, QMHP provided the client with verbal praise and encouraged her to continue keeping her hands and feet to herself when interacting with her peers. The client responded well towards Therapeutic Day Treatment services as evidenced by positive engagement throughout the day. During morning observations, the client was receptive to redirection and agreed to remain focused by following directions while eating breakfast with her peers. During midday observations, the client was unreceptive to redirection by becoming angry and stating, “leave me alone!”. The client was then able to calm down and agreed to communicate/respond to authority in an appropriate manner and to remain focused while watching a movie. During afternoon observations, the client agreed to continue keeping her hands and feet to herself when interacting with her peers while cleaning up the room.
